Researchers at Cincinnati Children’s Hospital Medical Center in the United States report they have identified a targeted therapy that substantially reduces the initial progression of Non Alcoholic Fatty Liver Disease.
Nonalcoholic fatty liver disease, or NAFLD occurs in every age group but people in their 40s and 50s who are at high risk of developing heart disease are especially affected by it. For the first time, researchers discovered the occurrence of a protein called cdk4 at higher levels in mouse models and human patients with non-alcoholic fatty liver disease. Also, in mice, when the protein was blocked by administering drugs, it notably reduced development of a condition known as hepatic steatosis – the first stage of the disease.
“This is the first study to show that cdk4 triggers development of NAFLD and that inhibiting this enzyme can both prevent and reverse the first step of the disease,” said Nikolai Timchenko, PhD, senior author and head of the Liver Tumor Biology Program at Cincinnati Children’s.
In NAFLD, liver cells have extra fat build up that’s not caused by alcohol. Liver normally have some fat, but if 5-10% of its weight is fat, then it is classified as fatty liver.
Hepatic steatosis (discussed earlier) can progress to a condition called non-alcoholic steatohepatitis (NASH) and eventually liver cirrhosis or liver cancer.
People who are overweight or obese or those with diabetes, high cholesterol, or high triglycerides are at greater risk of developing NAFLD. Poor eating habits and rapid weight loss can also contribute to the condition.
But, some individuals can develop the condition without these risk factors.
It is estimated that up to 25% of Americans have NAFLD.
Prof. Timchenko says new treatments for NAFLD are needed since there are no safe or effective treatments available besides weight loss and lifestyle changes. Although, new treatment alternatives are being tested in clinical trials, studies have revealed serious side effects from these treatments.
For the study, researchers turned to mice put on a high-fat diet. They call these mice one of the best biologically pertinent animal models for the condition. They say these rodents mimic the major steps of NAFLD development in humans.
Researchers found that the increase in cdk4 triggers a chain of events through certain pathways that result in hepatic steatosis, fibrosis, and hepatocellular carcinoma (HCC or liver cancer).
Repeated testing – including testing of donated liver tissues from human patients – showed a link between high levels of cdk4 and non-alcoholic fatty liver disease in humans and mouse models. Mice that were bred to not exhibit high levels of cdk4 didn’t develop the early stage of NAFLD, authors say.
They also found that using inhibitors flavopiridol and PD-0332991 to blocked cdk4 in mouse livers with existing steatosis reversed it.
The researchers say that the essential finding of their study was to identify the advancement or activation of cdk4 as a crucial event in the development of NAFLD.
They suggest this finding – along with further study with cdk4 inhibitors – will probably offer strong proof to support use of cdk4 inhibitors as a treatment that prevents the liver from developing steatosis, and even reverse existing steatosis.
“Both of the cdk4 inhibitors we tested are approved by the FDA and in clinical trials for liver cancer, so it should be possible to initiate clinical trials for NAFLD with these drugs soon,” says Prof. Timchenko.
The study was published in the journal Cell Reports.

Eating green leafy vegetables in larger portion daily may lower your risk of developing liver steatosis, also known as fatty liver disease, according to a recent study, published in the journal PNAS. Researchers from Karolinska Institutet in Sweden found a larger intake of inorganic nitrate, which occurs naturally in various types of vegetables, decreases build up of fat in the liver.
“When we supplemented with dietary nitrate to mice fed with a high-fat and sugar Western diet, we noticed a significantly lower proportion of fat in the liver,” said Mattias Carlstrom, Associate Professor from the Karolinska Institutet in Sweden.
“No one has yet focused on nitrate, which we think is the key. We now want to conduct clinical studies to investigate the therapeutic value of nitrate supplementation to reduce the risk of liver steatosis,” he added.
Nonalcoholic fatty liver disease (NAFLD) is a condition in which fat builds up in the liver. Between 30 and 40 percent of adults in the United States are affected by this condition.
Being overweight, obesity, and metabolic risk factors are believed to cause NAFLD. In Western countries, NAFLD is one of the most common causes of chronic liver disease. There is currently no approved treatment for the condition, which can progress into life-threatening conditions such as cirrhosis and liver cancer.
Mattias Carlström, an associate professor in the Department of Physiology and Pharmacology at the Karolinska Institutet, and colleagues conducted a study on mice to study the effects of supplementing a high-fat, high-sugar Western diet with dietary nitrate, reports Medical News Today.
The mice were divided into 3 groups, where each group received a different diet. The high-fat diet group received a diet that’s equivalent of a Western diet, the second group was given a high-fat diet with nitrate supplementation and the control group was fed a normal diet.
As expected, the rodents in the high-fat diet group gained weight and fat mass, and they also had higher blood sugar levels. However, the group that received nitrate along with a high-fat diet had significantly lower fat and blood sugar compared to the group that received only high-fat diet. These mice also had lower blood pressure and better insulin sensitivity compared to the high-fat only diet group.
Scientists haven’t yet identified the exact compounds that make leafy greens so healthful, explained the researchers. “No one has yet focused on nitrate, which we think is the key,” Carlström said.
“We now want to conduct clinical studies to investigate the therapeutic value of nitrate supplementation to reduce the risk of liver steatosis. The results could lead to the development of new pharmacological and nutritional approaches.”
While more studies are needed to identify the compounds responsible for the healthful properties of green leafy vegetables and to confirm that nitrate is crucial for liver health, the researchers advise people to eat more green leafy vegetables.
Calstrom says you don’t need to consume large amounts of these vegetables to get the protected effects, but only around 200 grams per day is sufficient. “Unfortunately, however, many people choose not to eat enough vegetables these days,” he says.

In all, the researchers assessed the dietary habits and health records of 25,994 women. All the participants were healthy at the study onset. The women were followed for 12 years.

As far as exploring the impact of Mediterranean diet in a U.S. population, this was the first long-term study to investigate the diet’s impact on CVD.
“[H]igher [Mediterranean diet] intake was associated with approximately one-quarter lower risk of CVD events over a 12-year follow-up period,” the authors wrote.
They also found that varying degree of metabolites associated with inflammation accounted for 29% of the reduction in CVD risk.
Insulin resistance and glucose metabolism accounted for 27.9%, BMI for 27.3% and blood pressure accounted for 26.6%.
“In this large study, we found that modest differences in biomarkers contributed in a multifactorial way to this cardiovascular benefit that was seen over the long-term,” explains corresponding author Dr. Samia Mora.
Once again, the Mediterranean diet has won over other eating patterns in a study.
Doctors normally suggest that a person should fast for several hours before taking blood test for diabetes. Now, a new study suggests otherwise. According to the study, this approach may do more harm than good for people with diabetes.
Diabetics tend to have higher levels of “bad cholesterol” or low-density lipoprotein (LDL). These high levels of LDL can cause too much fat buildup in the arteries.
For this reason, doctors may suggest that these people undergo normal blood cholesterol tests.
In order to get an accurate result, it is recommended that people abstain from taking any food or drink except for water before a blood test.
However, more and more studies are suggesting that the ‘fasting part’ may be skipped, fasting before a blood test may actually be harmful in some cases.
Researchers from Michigan State University in East Lansing say that for people with diabetes who take insulin or sulfonylurea (an antidiabetic drug widely used to manage type 2 diabetes), fasting before a blood cholesterol test may cause low blood sugar, also known as hypoglycemia.
For their study, the researchers worked with 525 individuals with diabetes who attended an endocrinology clinic in Michigan. Participants were asked to fill in a 2-page survey, and only those who featured all the relevant data were considered for the study.
After analyzing all the data, the team found that diabetics were more likely to experience something called Fasting-Evoked En route Hypoglycemia (FEEHD) if they underwent a blood test after fasting.
FEEHD is a condition when blood sugar levels become extremely low (under 70 mg per deciliter), and may cause dizziness, confusion, and faintness. This can cause the person to get into an accident if he or she is driving to the clinic where the test is scheduled to take place.
Study author Saleh Aldasouqi explains that hypoglycemia is a problem that’s overlooked, but is observed in diabetics who go to the clinic for a blood test after skipping breakfast. These people continue to take their diabetes medicines without eating anything, which results in low blood sugar, he said.
Aldasouqi says that their new motto is ‘Feed not FEEHD.’
He also points out that doctors know that having food before drawing blood for cholesterol test is not likely to impact relevant data. Therefore, eating food before going for a test may actually be a better choice rather than not eating and potentially fainting on the way to the clinic.
He explains that the notion of abstaining from food before a blood test comes from old guidelines that were set during the 1970s, which are no longer used by specialists in Canada and most European countries.
He suggests that the US-based specialists need to reconsider their old guidelines in order to better serve the needs of their patients.
The study found that only 35% of the participants had been advised about how to prevent a FEEHD before undergoing a blood cholesterol test.
Aldasouqi says they encourage patients who are going for a lab test to ask their doctor if it’s really necessary to fast, and if so, what would be the best to way to handle their diabetes medications during that period of fasting.

Melissa G. Hunt, the first author of the study, believes this is the first ever study on Facebook, Snapchat, and Instagram use. Previous studies, she says, had either limited range or relied on unrealistic situations, like observing participants for short periods in laboratory settings. “Here’s the bottom line. Using less social media than you normally would leads to significant decreases in both depression and loneliness. These effects are particularly pronounced for folks who were more depressed when they came into the study,” Hunt said.
The researchers picked Facebook, Snapchat and Instagram for the focus of their study because these social media platforms are most popular among undergraduates.
The team recruited 143 undergraduates at the University of Pennsylvania. Each participant completed a survey to determine mood and well-being at the study’s onset, and provided a week’s worth of baseline social-media data by sharing screenshots of their iPhone battery.
For the next phase, the participants were randomly assigned to a control group, where users had to maintain their regular social-media behavior, or an experimental group that had fixed usage time for Facebook, Snapchat, and Instagram to 10 minutes each per day.
The participants’ smartphone data were made available to the researchers over 3 weeks. The individuals also completed surveys that investigated a range of factors, including their depression, anxiety, loneliness, and fear of missing out.
The results revealed significant decreases in depression and loneliness among the group who reduced their social media use. These effects were especially strong for participants who were more depressed when they joined the study.
“It is a little ironic that reducing your use of social media actually makes you feel less lonely,” says Hunt. “Some of the existing literature on social media suggests there’s an enormous amount of social comparison that happens. When you look at other people’s lives, particularly on Instagram, it’s easy to conclude that everyone else’s life is cooler or better than yours.”
Because the study only examined three social media platforms, it’s the study couldn’t determine if the results might also apply to other social media platforms. However, Hunt aims to examine this in future studies, one of which will investigate the impact of dating apps on college students.
The researchers published their study in the Journal of Social and Clinical Psychology.
